Getting Started With Your Treatment
To begin your telemedicine care journey, we require all new patients to complete two quick steps. Please follow the instructions below so our provider can safely review your history and begin your custom treatment plan.
Step 1
Complete the Telemedicine Consent Form. This form allows our provider to legally establish communication and discuss treatment options with you. Please read the Telemedicine Consent & Treatment Information block below, then fill out the form directly underneath it.
Step 2
Complete the Health Questionnaire. Once your consent is submitted, please fill out the second required form. This questionnaire gives us a deeper understanding of your complete health history so we can design the safest, most effective treatment plan for you.
